The contribution of primary health care to global health.

چکیده
In March 2010, UN-Secretary General Ban Ki-moon challenged member states to increase their efforts to achieve the Millennium Development Goals (MDGs): ‘the lack of commitment increases the likelihood of a failure’. Nevertheless, there are positive signs: when it comes to eradication of extreme poverty, important progress has been made. Access to basic education, equity between men and women at the level of basic and secondary education, and access to water, show clear improvements. However, the picture is not that hopeful in relation to the health-related MDGs which include: reduction of child mortality and maternal mortality, tackling the HIV/AIDS problem, and preventing malaria and other diseases. Almost 8 million children yearly do not reach the age of 5 years and these problems are concentrated in Africa and South Asia. Progress is possible, as demonstrated by the countries in Latin America, where child mortality decreased between 1990 and 2008. The goal to reduce maternal mortality by 75% between 1990 and 2015, is likely to be achieved in only 23 countries, with Sub-Saharan Africa remaining the biggest problem. New health problems arise in developing countries: the noncommunicable diseases (such as diabetes and heart failure) are increasing and may seriously hamper the realisation of the MDGs.

The current system of health technology development is characterised by multiple misalignments. The “supply” side (innovation policy-makers, entrepreneurs, investors) and the “demand” side (health policy-makers, regulators, health technology assessment, purchasers) operate under different – and conflicting – logics. The system is less a “pathway” than an unstable ecosystem of multiple interacti...

Pay-for-performance (P4P) is the provision of financial incentives to healthcare providers based on pre-specified performance targets. P4P has been used as a policy tool to improve healthcare provision globally. However, researchers tend to cluster into those working on high or low- and middle-income countries (LMICs), with still limited knowledge exchange, potentially ...
متن کاملUniversity of Global Health Equity’s Contribution to the Reduction of Education and Health Services Rationing
The inadequate supply of health workers and demand-side barriers due to clinical practice that heeds too little attention to cultural context are serious obstacles to achieving universal health coverage and the fulfillment of the human rights to health, especially for the poor and vulnerable living in remote rural areas. A number of strategies have been deployed to increase both the supply of h...
متن کاملCritical Global Health: Responding to Poverty, Inequality and Climate Change; Comment on “Politics, Power, Poverty and Global Health: Systems and Frames”
A recent article by Sol Benatar calls on the global health community to reassess its approach to twin crises of global poverty and climate change. I build on his article by challenging mainstream narratives that claim satisfactory progress in efforts to reduce poverty and improve health for all, and arguing that any eradication of poverty that is consistent with environmental sustainability wil...
